What to gather before submitting Form 776
Use the state’s current property-owner page to obtain the form. The copy linked there when checked September 13, 2026 carries an August 7, 2017 revision date.

Occupancy changes the possible result
The applicant attestation distinguishes three circumstances:
- Occupied for the required period, with no observed malfunction: the form describes acceptance as possible; an advanced treatment system also needs the specified continuing maintenance arrangement.
- A malfunction is found: it describes a permit/recommendation and a notice requiring corrective measures within 30 days.
- Unoccupied, or occupied for too short a period: it describes a permit/recommendation for a new system rather than acceptance of the existing one.
The form does not state how many days or months satisfy its occupancy requirement. Ask MSDH what applies to the property before expecting an acceptance document for a sale or occupancy change. Submission, fees and the document to request afterward
Contact the MSDH wastewater call center at 855-220-0192 for application assistance. The program publishes wastewater@msdh.ms.gov for documentation; include identifying property information. Form 776 prints a $125 inspection application fee, with an additional electronic processing charge. Because the linked form is dated 2017, confirm the amount and the proper application route with the call center before payment. For an advanced treatment system, also resolve the required brand-specific inspection and maintenance qualifications. Do not assume that a general pumping receipt supplies those documents. Ask for the inspection copy signed by the environmentalist. Page 4 says MSDH supplies that copy to the applicant; incomplete requests are not processed. Retain the signed result with the submitted history and any corrective notice so a later reader can distinguish your supplied facts from the agency’s findings.
How we checked the request
LSL compared the current MSDH program and property-owner pages with all four pages of the linked Form 776 on September 13, 2026. The preparation table groups its actual requirements; no system was inspected and no soil suitability, repair design or future performance was determined.
