What can stop the application at intake
DOH-Hillsborough says it will not process an application without the property street address and folio/property ID. Put the owner as applicant and the abandonment contractor as agent, complete the applicable application fields, and provide the site plan. The linked DH4015 application is a general construction-permit form with an Abandonment checkbox. Do not mistake its other project options for additional applications automatically required for abandoning one tank.
There is a drawing-instruction conflict worth resolving before paying for drafting. The abandonment-specific webpage says the site plan need not be to scale, but must show buildings, streets and the tank location. The generic application’s attachment instructions say drawn to scale, and the separately supplied site-plan grid prints a scale. Ask intake which drawing it requires for this abandonment, identifying the specific form versions. Do not assume that every generic new-system attachment applies unchanged to this narrower job.
An authorization letter is required when the applicant’s agent is not a licensed septic-tank contractor or a contractor licensed under Chapter 489. Assign the pumping evidence and inspection call
Before work begins, ask the contractor who will supply each of these items and how you will receive the resulting record:

The receipt and empty/dry statement are alternatives in the local instructions, not two documents required for every tank. Neither alone proves that the required inspection occurred.

Inspection timing is not an instruction to leave a hole open
The local page directs the responsible contractor, plumber or homeowner to call 813-307-8059 for inspection after the tank has been crushed and filled with suitable material. It calls for flags at the street and tank location and explicitly says not to leave an open hole. It warns that failing to request the inspection can result in a citation or fine. For your retained file, request the issued permit, the applicable pumping evidence, inspection result and any correction or completion record the office requires. A contractor’s invoice, sewer connection or restored yard does not by itself answer whether the abandonment permit’s requirements were satisfied.
