Check the form’s stage before using it

The directory labels its link “Additional Information or Revision,” but the one-page form itself says the permit has not been issued yet. It asks for the submitted items and page numbers being replaced, with separate departmental decision fields.

If the permit is already issued, tell Building staff that fact and confirm the correct submission route/form. Do not assume the directory’s shorter label overrides the document’s explicit stage. A response from one discipline also should not be copied into another department’s approval space.

Closeout has its own documents and review time
