Pasco’s linked septic-repair packet does not treat a repair sketch like a new-system site plan. Its repair instructions explicitly say the plan need not be drawn to scale—but it must still show the property and system information needed for review. Before preparing an application, confirm the currently accepted form with the Pasco onsite-sewage program. The website currently links this packet, but its embedded DH4015 form is marked 08/09. The June 2025 upload folder is not evidence of a new form revision.
A repair sketch still needs specific information
LSL grouped the packet’s repair-only list into the following cross-check. This is a preparation aid, not a design or an approved drawing.

The packet contains both a grid sheet and instructions for other application types. Do not overlook the repair-specific subsection because the grid looks like the whole assignment. Equally, “not required to be drawn to scale” does not mean locations and dimensions can be guessed. Ask the office which current drawing and supporting information it requires for the proposed repair.
The distances used to identify nearby features on a drawing are not automatically the setbacks governing a design. LSL has not adopted this older packet’s numerical separation rules as a current determination for a property.
The owner does not create every required document
The packet cover separately calls for a pump-out certification completed by a registered/licensed septic contractor, a property-appraiser printout and a site plan. A homeowner’s description of a pump-out is not the specified contractor certification.
It also says water-use data may be requested for a building connected to a public utility: 12 months for residential use and 18 for commercial use. If requested, obtain the utility history rather than substituting one recent bill. Sewer availability can change the question the permitting office must resolve; a nearby line is not LSL confirmation that connection is required or available.
Confirm the form and assessment before submitting
The current program page says repairs require permits and explains that DEP administers the program while county health offices continue permitting and inspection. It links the current location finder for local contact information.
The packet cover lists $340 and warns that additional services can carry other fees. Treat that as a statement in the linked packet, not a verified final charge or a repair-cost estimate. Ask the Pasco office to confirm the accepted application version, submission route and assessment for the actual scope.
