These are separate programs on the City assistance page, not three names for the same discount. Confirm the account’s actual provider and services before applying.

Prepare the documents the application actually requests

The live page says households within the 50% area median income limit are eligible to apply and warns that HUD limits change annually. Follow its current income-limit link for the applicable household size; an old table is not a current eligibility determination.

The first page of the 11-page CAP packet lists these application materials:

  • Completed and signed water/wastewater application and certification statement.
  • Signed privacy policy and release authorizations for the applicable adult household members.
  • Identity-verification form and government-issued photo ID.
  • The most recent water/wastewater bill or new-account information.
  • Current proof of income from all sources/accounts: its examples include six weeks of paystubs, the latest Social Security/SSI benefit letter, pension statements, and—for self-employment—a year-to-date profit-and-loss statement plus the last two years’ tax returns.

Use the packet’s instructions for your household rather than treating those examples as a substitute for the City’s review. Its wording varies between “over 18” and “18 and older”; ask application staff which signatures and supporting forms it needs if a household member is exactly 18.

The checklist says the application does not need notarization or witnessing. The identity-verification page also has a clearly marked section for a City representative. Use the current submission route and read the privacy choices

The live page offers an online application and download. The packet’s checklist lists Housing & Community Development at 2555 E. Hanna Ave., Tampa, FL 33610, or fax 813-274-7945. Its privacy-policy text separately retains an older W. Lemon Street address for changing a disclosure choice. The packet explains information verification, disclosure choices and public-records considerations. Do not assume that every submitted document is confidential or that every field is publicly disclosable. Read the policy and ask the City about applicable protections before transmitting sensitive information. Keep your own copy privately; do not put income, Social Security or account information into a shared checklist or the webpage’s public feedback box.

Approval starts a conservation deadline and a renewal obligation

The application certification says to complete the conservation requirement within 90 days of receiving the kit to retain the base-charge waiver. Record the kit’s receipt date—not just the application date. The live page describes a survey online or by mail; the packet also lists workshop and in-home-assistance options. Annual recertification through the Tampa Housing Department is separate from that conservation step. The certification also requires notice of eligibility-information changes and a new application if the property address changes. A first-year approval does not settle those later obligations.

Keep the City’s decision, the effective bill information it provides, conservation completion and the annual recertification response. If a final notice or unpaid balance remains, address it with Utilities separately rather than assuming the CAP file resolved it.
