For this limited set of rows, LSL’s reusable check is $110 + $30C + $100D + $60S, where C, D and S are the City-assigned counts of curb cuts, driveway inspections and sidewalk items. Do not assume a sidewalk’s charge is per foot or that repeated visits are included. Ask Public Works how it classifies and counts the actual work.
Match the scope to the application before using a subtotal
The City form asks for an address or legal description, work description, start and completion dates, and a proposed-project sketch. Its departmental section separately records fee components, project approval and payment. Preparing or submitting the form is not approval.
The fee page distinguishes single-family work from industrial, commercial and multifamily work. It also lists a separate underground-utility permit calculation: a $110 base plus 2.5% of the constructed improvement value.
