To stop a City of Monroe water account, plan for at least one business day’s notice. The City-linked discontinuation form says a completed request must reach Customer Service that far ahead; same-day requests are processed for the next business day. Reach the right Customer Service office

The current Utility Billing instructions give two contact routes: visit 1401 Stubbs Avenue or call 318-329-2220. For termination, the page asks for identification, a forwarding address and a phone number; it specifies the last four digits of the Social Security number for phone identification.

The downloadable form still prints 316 Breard Street. Use the current billing page for the office location, and confirm the submission route before sending personal information. The older address is a reason to check instructions, not evidence that the entire form has been withdrawn.

An online termination form is also linked from the City site. Its visible fields include name, service address, account/customer number, last four SSN digits, phone, email, termination date and forwarding address. LSL inspected the loaded form but did not submit a request or verify its processing.

Choose a stop date with the notice period in mind

The PDF distinguishes receipt of a request from its requested service date. It also says a weekend or City-holiday stop date moves to the following business day.

These examples apply the published wording to an ordinary week with no City holiday; the City must confirm the actual appointment and any cutoff time:

A mailing date is not a receipt date. Ask Customer Service to confirm when the request arrived and which stop date it accepted, especially around holidays. The published form does not give a guaranteed hour of shutoff. Timing instructions

Keep the service address and forwarding address separate

The service address identifies the account to discontinue. The new mailing address is where you want future account correspondence sent. The paper form has separate spaces for both, plus account name and number, phone, requested date and signature.

It also distinguishes the named residential customer from someone acting for that customer, and a commercial owner or representative. If you are acting for another person, ask what authority or identification the City requires; checking a relationship box is not independent proof of authorization.

The online version includes a generically labeled “Single Line Text” field. Its purpose is not explained in the visible form; ask staff rather than guessing or entering extra sensitive information. Use only a City-confirmed route for personal data.

Decide how the final bill should be paid

The paper form includes an initial box for customers who pay by bank draft and do not want the final bill drafted. That is a specific instruction to discuss with staff, not evidence that closing service automatically cancels every payment arrangement.

Before leaving or ending the call, ask:

  • What termination date was accepted, and how will completion be confirmed?
  • Will the final bill be drafted or require another payment method?
  • Where will the final statement be sent, and how should any deposit balance or credit be checked?

Neither the request page nor the form establishes your final reading, amount owed, refund or closed-account status. Keep the City’s confirmation and final statement privately. If you are opening a different Monroe account, the service-start guide covers that separate task.

On September 13, 2026, LSL compared the current billing instructions, the loaded online termination form and the complete one-page discontinuation PDF, including its rendered fields. We replaced the former inspection-style ledger with the actual notice-period examples, address discrepancy and final-draft choice. We did not inspect a private account, submit a form, contact staff or verify a completed termination.
