The form’s staff-only section separately provides for a deposit and existing bad debt, if any. That is another reason not to advertise $85 + $25 as a guaranteed $110 start-service quote. It does not establish that a particular applicant owes a past balance or either current amount.
Before sending identity documents
On the City page, open Residential Water Service to see the current call route, ID requirement and displayed deposit. Ask the representative for the accepted form and the actual fax, mailing or online destination before transmitting private documents. We verified the public instructions, not a working remote application or an individual account.
Keep identity and account information private. A publicly shared moving checklist needs a status such as “documents accepted,” not copies of personal identifiers. An application receipt establishes submission; it does not establish that water is on.
The form explains why a turn-on visit can fail
Its operational note says faucets and connections must be off, service staff cannot enter the building, and water will not be left on if the meter indicates flow. Before the visit, resolve any known uncontrolled-flow concern with the appropriate professional and follow the City’s site-readiness directions. Do not operate the City meter yourself or expect field staff to enter the premises to turn off fixtures. After the visit, ask for the actual service result if water is still unavailable rather than treating the requested date as a completed activation.
A PayStar login is a different kind of account
The City’s PayStar guide begins payment lookup with an existing utility account number and explains how to create a web login. Those instructions do not say that a PayStar login opens a new water-service account. Keep the application confirmation, any payment receipt and the field activation result distinct when following up.

The old form’s office address and missing revision date remain visible limitations.
