Zephyrhills’ Public Works driveway form is specifically headed Construction Within Public Right-of-Way. Its most useful distinction is between information the applicant supplies and checks reserved for the City. A filled form is not an approved driveway design.
Prepare the sketch and application together
The driveway application asks for a sketch and the following project details. Match the sketch to the fields so reviewers are not left to infer the scope from “replace driveway.”

The form warns that work exceeding the description may require additional drawings or permits. Revise the application when the scope changes rather than relying on a sketch for a smaller job.
Leave the City’s review fields to the City
The office-use section contains surface-thickness, length, width, sidewalk, accessibility, expansion-material, parking-pad, flare, visibility and setback checks. These are not a stand-alone engineering specification. Do not copy their printed dimensions into a construction contract as proof that the site is approved. The form reserves additional work descriptions and the approval signature for Public Works.
It also says driveways must not interfere with existing stormwater treatment or conveyance and that the City is not responsible for driveway maintenance or repairs. Ask the reviewer how drainage, sidewalk continuity and utility conflicts affect the proposed work before setting a pour date.
Signing and filing are different from online submission
The public page presents this as a print-only form. It gives a return address of City of Zephyrhills, 5335 8th Street, and fax 813-780-0005. It says the application is void without proper identification and a signature witnessed by a permit technician or notary. Confirm the current accepted delivery method; typing into the web fields is not a filing receipt.
Why the general building form still matters
The separate general permit application says driveway work on public roadways is not an over-the-counter transaction and needs right-of-way review. It does not replace the Public Works scope or establish that every driveway needs the same building documents.
That downloadable form also carries old notice-of-commencement amounts of $2,500 and $7,500 for A/C work. They do not match the current state permit provision, which uses a greater-than-$5,000 general notice-copy threshold and a separate exception for repair/replacement of existing heating or air-conditioning below $15,000. Confirm applicable notice obligations and timing with the City; do not use the old amounts as current instructions.
