Check the form year before gathering financial records

On September 20, 2026, the County page still linked the application revised May 30, 2025. Its income table is effective April 1, 2025, and its document checklist names earlier tax years. Ask the County for the currently accepted form, income limits and document periods; do not treat the old table as a verified 2026 eligibility calculator. Linked application

The published threshold conditions concern owner occupancy, income, current insurance, current property taxes and a Health Department citation. The form also recognizes an EPA order to connect. Describe the order actually issued for the property and ask which route applies. Do not seek an unnecessary citation merely to fill a checklist.

Separate your costs from the financed work

The application places permit fees, inspection fees, matching funds and work outside the property’s parcel with the applicant. That makes “approved for assistance” different from “nothing to pay.” Use this short comparison when the County explains a proposed award:

Prepare private documents without sending originals

The application asks for household identity and applicable income records, insurance, mortgage information, taxes and the health or EPA order. It says not to send originals. Keep the submitted copy and receipt privately; do not put identity, bank or Social Security records in a public repair log.

Match names and the property across the records. Explain legitimate differences with supporting documentation rather than altering a record. Confirm the current secure upload or delivery route on the County page, which lists the HSTS mailing address as 175 S. Main Street, Room 207, Akron, OH 44308. Its general department footer gives a different address. Call 330-643-8013 before arranging a visit or sending sensitive documents.

Approval leads to bidding, not immediate construction

The dated application describes County bidding after approval and a contract with the lowest responsive, responsible bidder. Owners must provide access for the process. It calls the program nonemergency, says applications are good for six months and may need updating, and expressly excludes using it to pass a point-of-sale inspection. A preferred contractor’s estimate is not the County contract. Application pages 2–3

Do not delay an urgent health response while waiting for financial assistance. Ask the responsible health authority which interim actions apply; this article does not prescribe sewage repairs.

Read the five-year obligation before signing

The application describes equal forgiveness on mortgage anniversaries, continued tax and insurance obligations, and adding the County as an additional insured. Sale, transfer, loss of primary-residence use or death can make the unforgiven amount due. Its acknowledgment also addresses refinancing and the eventual mortgage release.
