Green publishes different starting contacts for stormwater quality and flooding. Choose the route from what you observed; a suspected cause is not a diagnosis.
Quality concern or water accumulating?
The City’s Storm Water Division page gives this distinction:

The same page’s general contact card lists 330-896-6607 for the division; that does not erase the separate quality-reporting number in its instructions. If both problems are present, describe both and ask the receiving office whether another department also needs the report.
The City specifically warns against putting chemical solvents, paint, vehicle fluids, leaves or pet waste into storm drains. An unknown sheen or color should be described as observed, not labeled with an unverified chemical name.
The online form is location-based
Green’s Make a Service Request form requires a name, phone, email and an identifying location. Its request choices include Roadway Flooding and Other. It also offers a picture upload, additional comments and an email-copy option.
For a non-urgent roadway report, supply the nearest street address or intersection and identify which side or section of the road is affected. Add when you saw the condition and whether it persisted after rain. A safe existing photograph can show the location and obstruction without requiring the reader to enter water or traffic.
Do not choose Roadway Flooding merely because any part of a private yard is wet. Describe the actual setting; if the category is unclear, use the published contact to ask where it belongs. Selecting a form option does not decide whether a public drain, private pipe, grading or another condition caused the problem.
Do not use a form receipt as a repair commitment
Keep the submitted description and any reference number or reply so a later conversation can identify the same location. Ask what the office found and which party handles the next action. A copied email is evidence of the request, not of an inspection or scheduled repair.
The division’s contact card separately lists 330-896-6610 as its emergency phone. For immediate danger, use emergency services rather than waiting for an online request. The reviewed form does not promise an urgent response.
