Describe what you observed before naming a suspected source

The form separates incident date, time, location and description from the Suspected Source field. It also invites weather details, photographs or video, witnesses, actions already taken and authorities already contacted. Form fields

  • Observation: Brown water was flowing into the street inlet at the stated location and time; rain had recently stopped.
  • Unknown: The material’s identity, where it originated and who was responsible have not been established.

This is an example, not a reported West Monroe incident. The observed color or a nearby worksite alone does not identify a pollutant or prove who caused it. Keep any possible-source explanation qualified and separate from what you saw.

Record only what you can observe safely. Do not enter water, handle unknown material or trespass to fill a field. An immediate hazard needs the appropriate emergency response, not a delayed document submission. A blank action field does not instruct a resident to attempt cleanup.

Confirm where the report goes

The resource page provides the City’s general number, 318-396-2600. Ask for the current Environmental Compliance reporting route, where to send the completed report and media, and what acknowledgment to retain. The document offers an anonymity preference alongside reporter-contact fields. Ask the City how that preference and follow-up are handled; LSL cannot promise confidentiality or an investigation outcome from this wording alone. Share witness contact information only with permission.

LSL read the complete resource-page text and visually inspected the one-page PDF on September 13, 2026. The PDF contained no interactive fields, document JavaScript or link/button annotations when checked. This update replaces a repetitive submission ledger with an observation-versus-inference example and the actual delivery limitation. No incident, pollutant source, private report or City response was investigated.
