Do not mail your only receipt or original photographs with a JXN Water damage claim. The utility’s claim form requires supporting copies and says submitted materials will not be returned.

The claim route is for someone alleging that JXN Water damaged property while performing its work. It is not a finding that the utility caused a particular loss, and submitting a claim does not guarantee repair or reimbursement.

Where the form goes

The one-page form linked from JXN Water’s File a Claim page gives two submission destinations:

The email’s domain differs from the utility website, but this address appears in the utility’s own linked form; it is not an LSL referral. Reopen the official download and confirm its current instructions before sending private documents.

For a current service problem, the claim page also lists 601-500-5200 as JXN Water’s 24/7 customer-service line. Calling that number and submitting a compensation claim are different tasks. Keep any service-report reference, but do not assume a call submitted the signed claim or secured payment.

Separate the account of the loss from the amount requested

The form asks for claimant contact details, physical and mailing addresses, and an account number if applicable. Its loss section asks when and where the event occurred, how it happened and the amount of loss. Additional explanation sheets are allowed.

It also asks whether a claim was filed with an insurer, for contacts of witnesses relied on, and for an amount at which the claimant would settle. Those are separate questions. LSL does not calculate a settlement amount, interpret insurance coverage or decide who is responsible.

Describe what you observed and identify the supporting attachment rather than presenting an estimate as proof of cause. For example, a repair estimate can describe proposed work and price; it does not itself establish that JXN Water caused the condition. The claimant signs a certification that the submitted information and supporting documents are accurate.

An attachment index that keeps the evidence distinct

This is LSL’s optional organizing aid, not an additional utility form. Make one entry for each relevant copy, using a short identifier you can refer to in the explanation. Keep the originals and a duplicate of the submitted set privately.

Alongside each entry, note the attachment identifier, document date, relevant claim point and whether that copy was actually included. These labels are an editorial suggestion; JXN Water’s form does not prescribe LSL file names. Avoid posting account numbers, witness contacts or the claim bundle publicly.

Keep a copy of the signed form and the email or mailing record that identifies the submitted package. If more material is requested, record which version you send. A delivery record or acknowledgment is evidence of transmission or receipt, not a decision on liability.

The two sources reviewed do not establish a numerical response deadline or a required inspection-and-certificate sequence. We have removed that sequence from the earlier guide. Follow any case-specific request from the confirmed utility contact without treating it as a promised outcome.

If the issue is the amount of a water bill rather than damage during utility work, the separate JXN Water rate calculation addresses usage units and service charges. It does not establish eligibility for a damage payment.
